CHECKLIST OF REQUIREMENTS:
- Duly accomplished DMW Standard Employment Contract;
- DMW 雇用契約
– In cases when the Company Contract misses some of the DMW-prescribed provisions, an Addendum to the Company Employment Contract containing the missing provisions must also be submitted with the signature of both parties

- Copy of PASSPORT valid for at least six (6) months from the date of intended departure;

- Copy of valid RESIDENT CARD;

- Any proof of existing employment with the current employer to which the worker is resuming employment such as a current CERTIFICATE OF EMPLOYMENT, valid ID, or RECENT PAY SLIP;

- Sworn statement of the worker providing an explanation on how he/she was hired by the current employer;
- Copy of the Insurance Card (Kenkou Hokensho) provided to the worker

- Copy of valid passport of the authorized representative who signed the employment contract

- a. For Companies ONLY: Company Registration (Japanese Version) with English translation
b. For Sole Proprietorship ONLY: Tax Certificate with English Translation- 法人の場合: 登記簿謄本「履歴事項証明書」及びその翻訳
- 個人事業の場合: 納税証明書の写し及びその翻訳
- Other relevant documents as may be required

CHECKLIST OF REQUIREMENTS:
- Duly accomplished DMW Standard Employment Contract;
- In cases when the Company Contract misses some of the POEA-prescribed provisions, an Amendment to the Company Employment Contract containing the missing provisions must also be submitted with the signature of both parties;
- Copy of valid and appropriate visa and/or residence card;
- Copy of passport of the worker which is valid for at least six (6) months from the date of intended departure;
- Any proof establishing employment with the employer such as a certificate of employment OR pay slip;
- Sworn statement of the worker providing explanation on how he/she was hired by the current employer;
- Copy of the medical insurance provided to the worker;
- Copy of valid passport and resident card of the EMPLOYER;
- Original copy of the EMPLOYER’s certificate of employment issued by his/her company; and
- Letter of Guarantee of the employer submitted to the Japan Immigration

- Embassy Contract with English Translation
- Copy of VALID passport of the Worker
- Copy of Visa of the Worker
- Certificate of Employment given by the Embassy for the Worker
- Copy of Worker’s Embassy ID
- Sworn statement of the worker providing an explanation of how he/she was hired by the current employer.
- Copy of the Kenkou Hokensho (Health Insurance Card) provided to the Worker
