  2. Submit complete documentary requirements to Migrant Workers Office (MWO) – Tokyo, Japan thruogh POST MAIL:
    5-15-5 Roppongi, Minato-ku, Tokyo 106-8537
    Philippine Embassy, MWO-Tokyo


    IMPORTANT NOTES:
    Attach a return letter pack (Japan Post)
    FOR CONSULAR MISSION: PROVIDE TWO (2) SETS OF DOCUMENTS. (1 COPY AND 1 ORIGINAL)
  3. Register in the DMW E-Registration or DMW Online Services Portal (URL: https://onlineservices.dmw.gov.ph/OnlineServices/(S(a23zylkysvxnd5c53tb2lclv))/POEAOnline.aspx?ReturnUrl=%2fOnlineServices%2fBMOnline%2fBMProcessing.aspx)

Without existing records/not registered with DMW

CHECKLIST OF REQUIREMENTS:

  1. Duly accomplished DMW Standard Employment Contract;
    • In cases when the Company Contract misses some of the POEA-prescribed provisions, an Amendment to the Company Employment Contract containing the missing provisions must also be submitted with the signature of both parties; 
  2. Copy of valid and appropriate visa and/or residence card;
  3. Copy of passport of the worker which is valid for at least six (6) months from the date of intended departure;
  4. Any proof establishing employment with the employer such as a certificate of employment OR pay slip;
  5. Sworn statement of the worker providing explanation on how he/she was hired by the current employer; 
  6. Copy of the medical insurance provided to the worker;
  7. Copy of valid passport and resident card of the EMPLOYER;
  8. Original copy of the EMPLOYER’s certificate of employment issued by his/her company; and
  9. Letter of Guarantee of the employer submitted to the Japan Immigration

CHECKLIST OF REQUIREMENTS:

  1. Embassy Contract with English Translation
  2. Copy of VALID passport of the Worker
  3. Copy of Visa of the Worker
  4. Certificate of Employment given by the Embassy for the Worker
  5. Copy of  Worker’s Embassy ID
  6. Sworn statement of the worker providing an explanation of how he/she was hired by the current employer.
  7. Copy of the Kenkou Hokensho (Health Insurance Card) provided to the Worker
